SMBC Youth Ministry uses one release form for the year, unless a specific trip location or organization requires their specific form. You can download the form below, complete it, and return it to the church office. Forms will be kept on file for the calendar year.
If you are interested in being part of the leadership with our students, you can fill out the application below. Once it is completed, please bring it to Brian and you will be contacted for a follow-up conversation to discuss if you are suited for any roles that might be available to you.
Volunteer Application WITH Background Check
Students who are interested in serving with children in capacities such as VBS must fill out a Volunteer Application. Once the form is completed, please return it to the office or to the appropriate ministry representative.
